Anterior Lumbar Interbody Fusion (ALIF )
Indication :-
- Spondyloleisthesis-
Isolated alif in grade 1 leisthesis
Grade 2 onward leisthesis-posterior fixation to be accompanied.
- Iatrogenic Segmental Instability
Hypermobility of a lumbar-motion segment requires rigid fixation to improve fusion rate.so used in combination with posterior procedure
Advantages :-
- Better visualization of disc space
- Safer graft placement
- Intact dorsal musculature
Disadvantages:-
- Posterior structures / roots not visualized
- Risk of venous thrombosis
- Retrograde ejaculation
- Operative time is long
- Blood loss is more as compare to other fusion
- Hospitalization time is longer
- Total costs of surgery is more
- Risk of damage to sacral nerves by the anterior approach to L4-5-S1
